The following applies if you are coming to hospital for a surgical or planned admission:
Please complete the pre-admission form (ensuring that there are two contact names and telephone numbers included) and return it seven days prior to your anticipated admission date, via post, addressed to:

Confirmation of admission time:
Please contact the Ward Clerk on 5761 4363 between 4pm and 6pm the day before your procedure to confirm your admission time and place.
You will be informed of the time from which you are to fast (that is no food or fluids) by the ward clerk when you confirm your admission time (unless you have special instructions from your doctor).
Please make sure you do not eat or drink for the recommended time prior to surgery.
You should not smoke or consume alcohol for 12 hours prior to your surgery.
If you are coming to hospital for an emergency or medical admission:
If your admission is unplanned you will need to bring with you any relevant documentation provided by your doctor at the time of consultation.
Please bring any medications that you are presently taking with you.
