What we do

The main role of the pharmacy is to ensure safe, effective, and appropriate use of medications.

We offer range of services that include:

  1. Medication reconciliation on admission and discharge
  2. Review medication charts (including high-risk medications) to prevent or minimise side-effects and drug interactions
  3. Work collaboratively with medical team to ensure safe medication practice
  4. Provide education and information on medications to patients and families
  5.  Liaise with your local community pharmacy at discharge for medication administration devices (eg. Webster packs) and with any changes to your medication regimens
  6. Educate and train under-graduate pharmacy students from various Universities
  7. Clinical services in the Urgent Care Centre (UCC)
  8. Support aged care facility linked with hospital
  9. Provide safe and efficient drug distribution system
  10. Education sessions on Pulmonary Rehabilitation and Cardiac Rehabilitation

What do I need to bring when I go to hospital?

  • Your current medications in original packaging. Include inhalers, vitamins, supplements, patches, eye drops, insulin etc
  • Your updated medications list
  • Your allergy history

Do I need to pay for medications in hospital?

No, you do not need to pay for medications supplied during hospital admission.

Can I get my medications dispensed on discharge?

No. When you are discharged from the hospital your doctor will write a prescription which can be dispensed at your community pharmacy.
