District Nursing Service / Home Nursing Services
This service is jointly funded by the Victorian and Australian Governments HACC, CHSP & TAC.
The District Nursing Service’s major objective is to ensure high quality professional nursing care for people with short or long term health issues or disabilities, who choose to remain in their own homes. Professional nurses provide individualized care including:
- Clinical assessment
- Medications/injections
- Wound management
- Ongoing - monitoring to assist wellbeing
- Diabetes education/ maintenance
- Continence management
- Health education and promotion
- Referrals to allied health professionals and other services
Who is appropriate to receive District Nursing?
Predominantly the District Nursing Service provides services to older people, younger people, people with disabilities and carers of these two groups. Services may be provided to other people who require nursing care in their own homes at a cost and if resources are available.
Eligibility of each person will be individually assessed. This program is supported by funding from the Commonwealth and Victorian Governments under the HACC program.
